Notice of privacy incident

On November 14, 2017, Pedes Orange County (“Pedes”) learned that an unauthorized individual may have gained access to one of our databases. When Pedes learned of this, we immediately launched an investigation and started working with the unauthorized individual to destroy all patient medical information that was accessed without permission. Our investigation has determined that the database may have contained information related to the care some patients received, including their name, medical diagnosis, medical treatments, dates of medical service, and other treatment related information. Importantly, financial account information and Social Security numbers were not stored in the database.

At this time, we have no evidence that any patient information has been misused by the unauthorized individual. However, in an abundance of caution, Pedes is beginning to mail letters to the potentially affected individuals. Pedes has also established a dedicated call center for individuals to call with any questions.

Individuals who believe they are affected by this incident but who have not received a letter by January 26, 2018 are encouraged to call 844-551-1716, Monday through Friday, between 6:00 am and 6:00 pm Pacific Standard Time. To help prevent something like this from happening in the future, Pedes is conducting a comprehensive review of our security practices, procedures and safeguards.
